How can difficulty options be improved on?

In a game you usually get easy, normal or hard. Most of the times this simply means enemies do more damage to you. I know this differs game from game but how could a game add/change things to make a game more challenging but without increasing enemy damage? If you have examples of games that do, then please post them?
