How can difficulty options be improved on?

Possibly what they can do is just more enemies, smarter AI, or faster enemies, these possibilities are possbly endless. I don't know how games difficulty modes work. My friend once put me on Legendary on Halo 4 and to my surprised I kicked some major butt, when usually I die all the time on even Recruit.
